Care Conversations/ Dementia Care Coaching

A clear, calm place to start when memory loss is affecting your family

We begin with Unbiased Care Conversations. These are neutral, person-centered conversations grounded in listening and thoughtful assessment, not assumptions. This is not a sales pitch and not a one-size-fits-all checklist. It is a steady, respectful conversation that looks at the whole picture and leads to next steps you can actually use.

From this foundation grows Dementia Care Coaching. Coaching offers ongoing education, reassurance, and practical support as needs change over time. Rather than diagnosing or directing medical care, dementia coaching helps families understand behaviors, communication changes, and functional shifts, and supports them in responding with confidence and compassion in everyday life.

Together, care conversations and coaching help families move from confusion to clarity, and from feeling overwhelmed to feeling supported.

What “unbiased” means here

Unbiased means we begin with assessment and listening, not assumptions.

We take time to understand:

  • What you are noticing (and what others may be missing)

  • What day-to-day functioning looks like right now

  • Safety concerns (driving, falls, medication routines, wandering risk, cooking)

  • Communication changes and emotional well-being

  • Caregiver stress and household support

  • What the person values most and what helps them feel grounded

  •

Then we help you connect to appropriate resources based on your needs. That may include referrals to medical providers, community services, caregiver supports, dementia education, or activity and engagement programming.

What you can expect

A conversation that is:

  • Respectful and confidential

  • Practical and nonjudgmental

  • Focused on function, safety, and quality of life

  • Centered on the person, while also supporting the caregiver

You’ll leave with:

  • A clearer understanding of what may be going on

  • A short list of priorities (what matters most right now)

  • Suggested referrals and resources that fit your situation

  • Next-step options that feel realistic, not overwhelming

Optional: Add RCCT Screening for added clarity

If you choose, we can include the Reality Comprehension Clock Test (RCCT) screening as part of the process. The RCCT can help identify an individual’s current level of comprehension and functional ability and may reveal potential, previously unrecognized functional deficits that are easy to miss in everyday life.

This can be especially helpful when families are hearing mixed opinions like:

  • “They seem fine to me.”

  • “It’s just normal aging.”

  • “Something feels off, but I can’t explain it.”

  •

The RCCT gives you another piece of the puzzle so care and support can be better matched to what’s actually needed.

Who this is for

Unbiased Care Conversations are helpful for:

  • Family members noticing early changes and wanting to respond wisely

  • Caregivers feeling stuck, burned out, or unsure what to prioritize

  • Families navigating a new diagnosis and trying to understand what it means

  • Professionals supporting someone with dementia who need clearer insight into functioning and next steps

  • Anyone who wants a dignity-centered plan for care and engagement

How it works

  1. Schedule a conversation (in-person or remote, depending on availability)

  2. We listen and assess your concerns and current routines

  3. Optional RCCT screening to better understand functional level

  4. We summarize what we learned in plain language

  5. You receive next steps and referrals matched to your situation

  6. If desired, we can connect you to ongoing support, including the Gift of Purpose Activity and Life Enrichment Program

Our goal

To help you move from uncertainty to clarity, and from clarity to compassionate action.

Because when people feel seen, supported, and understood, better care becomes possible, and caregivers can breathe again.
